Your toilet may require a new rubber seal on the flush valve (the flush valve controls the water out of the toilet cistern and into the toilet bowl).
Sometimes it may require a new inlet valve (the inlet valve controls the water flowing into the toilet cistern).
Another common problem is the flush cone rubber (this is the rubber seal located at the rear of your toilet that joins the flush pipe from your cistern to the rear of your toilet pan). Old toilets may need replacing either due to cracks in the toilet or cistern, or the toilet is really old and requires a lot more water to flush than a new water saving toilet.
